Connected Healthcare has been a leading provider of home health services in the Portland, Oregon community for more than 12 years. While the organization has grown to meet the needs of the community (adding the hospice branch in 2015 and mobile Primary Care in 2024), our commitment to providing life-changing services has never changed. We are a locally operated organization with strong support of resources who continue to help us follow best practices and help our staff lead the way with providing quality care for our neighbors in the community. Our community continues to expand which will allow us to provide quality care to more of our neighbors in the greater area surrounding Portland, Oregon.
