Alzheimer’s Navigator™ guides you through the disease
A diagnosis of Alzheimer’s disease or related dementia
raises many questions for people with the disease and their caregivers. And
everyone’s situation is different. Alzheimer’s Navigator™ is designed to help
both individuals with Alzheimer’s and caregivers develop a customized action
plan of information, support and community resources. Alzheimer’s Navigator,
created by the Alzheimer’s Association, is an online assessment program that
helps persons with dementia and caregivers evaluate their needs, execute and
coordinate action steps, and be linked to Association chapter programs, local
community services and referrals. After answering a welcome survey, users will
find recommendations for additional in-depth surveys tailored to specific
needs. Responses to the recommended surveys help formulate an action plan
featuring suggestions, tips and strategies and links to materials and local
resources.
Visit alzheimersnavigator.org for more information and to
get started.
