Program Summary: Communication is a key element in providing quality care and services to both the individual living with Alzheimer’s/Dementia and their caregivers. We have brought together a wealth of industry expertise from the NE region of PA. We will discuss the communications/conversations that occur throughout the stages of this disease. Topics will cover pre-diagnosis, denial/acceptance, diagnosis, case management, health and nutrition, transitional care, grief/bereavement and early stage community support. Together we shall learn more about the importance of these conversations and how to better serve those that depend on us for the best practices in quality care.
