Support a Critical Virginia Budget Amendment

A new budget amendment in Virginia will establish a pilot program to provide dementia specific case management and plans of care for 100 participants at a cost of $300,000.

The Alzheimer’s Association states only 45% of people with Alzheimer’s disease or their caregivers report being told of their diagnosis. Further studies have found that primary care partners are often overwhelmed, ill-equipped, and under-resourced to provide comprehensive management or care coordination for individuals with dementia. Coordinated care programs using trained Dementia Case Managers embedded in the memory assessment clinics are desperately needed for successful dementia-specific care and would realize significant cost savings, decrease health care utilization, and improve health outcomes.

This pilot program would address these issues by focusing on critical areas such as:

  • Diagnosis
  • Team Care Planning
  • Home Visits
  • Dementia Case Manager Ongoing Contact
  • Scheduled Clinic Appointments
  • Plan of Care
  • Regular IDT Meetings
