$100 CEU Professional (includes CEUs) (Nurse, LNHA/CALA, NJ Public Health, Pastoral Care [Association of Professional Chaplains])
*Note: Special certificates are available for members of the National Council of Certified Dementia Practitioners. [Certified
Dementia Practitioner (CDP), Certified Dementia Care Manager (CDCM), or
Certified Alzheimer’s Disease and Dementia Care Trainer (CADDCT)]. Please register as the $50 professional. These qualified professionals should email kprendergast@alz.org, the day after the event to request a certificate. Certificates will be provided by email only if
attendance is verified. The participant must check-in at the general
